REQUISITE DOCUMENTS

We require your identity paper, your medical prescription and possibly your social security card or your insurance card.

As for the patients having a support (Through their employer, Social Security Fund, insurance or other…), required at the time of admittance.

IMPORTANT

Services should be paid to the clinic’s cash desk at the time of admittance excepting the patients who have a support.

Medical consultations and examinations will be done by the practitioner named by the medical head.

All request for further services (other than the FRE) will be invoiced in addition to the package agreed.
